Change of Address: Please fill out an operator update-change form and print and fax it to 307-777-6779.

Contract Operator: Please fill out a operator update-change form if interested in working as a contract operator or are currently working as a contract operator.

Facility Configuration: All public drinking water treatment, drinking water distribution, wastewater treatment and wastewater collection facilities must have a facility configuration form completed by your District Engineer.

Generic Sign-In Spreadsheet (ITR): Please fill out the information in the spreadsheet completely.  Use the operators' 5-digit ID numbers.

Individual Training Report (ITR): Print one copy of the ITR  form to take with you to your training. After you have filled out the ITR and the instructor has signed it, make a copy for yourself and mail or FAX the signed original to DEQ.

License Application:  After you've passed your test, fill out a license application, print it, and scan/email, fax or mail the application to DEQ. The Operator Certification Program Cheat Sheet details how many hours and years experience are needed for each license.

Operator Designation: The operator designation form  tells DEQ which operators are working for your system and must be completed by the individual legally responsible for the system.

Reciprocity Application: Operators with current certification in other states may request reciprocity from the State of Wyoming.  Please fill out one form for each reciprocal license. The Operator Certification Program Cheat Sheet details how many hours and years experience are required for each license in Wyoming.
