LQD Noncoal Forms, Bond Info, Reports, Handbooks and Publication Formats

Copies of all forms mentioned on this page are available from any of the LQD offices. Please note that some of the Forms posted below are designed to be printed on 8.5"X14" legal-sized paper and are indicated by an * in front of the Form Name (forms may be printed on 8.5"x11" inch paper if the document if formatting does not change except for the size).  Forms must contain an original signatures to be accepted by the LQD.  Please use blue ink for signatures when possible to ensure original documents are identifiable.  Any alteration of the forms WILL RESULT IN REJECTION. Additionally, a web based format is also provided for many forms. No additional software is needed to use these forms. They are generated by our server. Limited Mining Operations

For operators with Limited Mining Operations (LMOs) effective June 1, 2004, the Department of Environmental Quality is requiring that these operations have an Air Quality Division (AQD) Permit.  See "LMO General Application Instruction Memo" posted below for further information regarding AQD contact information applicable to LMOs.  Effective July 1, 2013, revised legislation regarding LMO's - see the enrolled act to see what changed [Download Revised Legislative Changes Here]
