LQD Coal Permitting
Copies of all forms mentioned on this page are available from any of the LQD offices. Please note that some of the Forms posted below are designed to be printed on 8.5"X14" legal-sized paper and are indicated by an * in front of the Form Name. Forms must contain an original signatures to be accepted by the LQD. Please use blue ink for signatures when possible to ensure original documents are identifiable. Any alteration of the forms WILL RESULT IN REJECTION. Additionally, a web based format is also provided for many forms. No additional software is needed to use these forms. They are generated by our server. The are designed to be completed on-line, printed and mailed to us.
